A Client Information printable gathers your client's basic information, such as their name, contact information, and a short questionnaire. The questionnaire includes yes and no questions about allergies, lash history, and more to give the Artist a better understanding of whether the client is a good candidate for the service. The client should fill out this form before the service and consultation, along with an Eyelash Extension waiver to protect yourself legally and inform the client about the service.
